Any graduate who needs a copy of their high school transcript should put her request in writing and mail the request to the school. Attached please find the transcript request form. 
 
Be sure to include:
 
Full Name and Maiden Name
Date of Birth
Year of Graduation
Address to where the transcript must be sent
Contact information (telephone number, email address, or home address)
 
 
There is a $4.00 application processing fee.  Checks may be made out to St. Agnes Academic High School.
